First Visit
Your child’s first visit
If paperwork has not been completed, please arrive 20 minutes early. If paperwork has been completed, please arrive 10 minutes before your scheduled appointment time to allow the nurse to get your child ready for his or her visit.
You can save time by completing your registration and forms online before you come in.
When you come to the office, please bring
Insurance card
If the card is not available yet, please tell the front office staff the name of the pending insurance.
Photo ID of the guarantor or guardian
Whoever is financially responsible for the visit.
Any previous medical records
Including immunization records and recent visit notes from a prior practice.

We believe in the effectiveness of vaccines
We believe in the effectiveness of vaccines to prevent serious illness and save lives. If you have any questions about vaccines, please talk to one of our providers who would be happy to answer your questions.
We do have a Minimum Vaccine Policy to remain a patient at our practice. If you would like to read more about vaccines, we recommend the Children’s Hospital of Philadelphia Vaccine Education Center.

For a newly diagnosed ADHD visit, please bring a copy of your child’s school evaluation to the appointment.
